Referring Providers

Thank you for your referral to OIO!

We accept patient referrals through several options:

• Electronic referrals through your EMR/EHR system (preferred)
• Download our referral PDF form here and fax it to 419-224-0015. A representative will then call the patient to schedule within 2 business days.
• Call us at our main scheduling line: 419-222-6622, Option 1

We take great care in scheduling referrals as soon as possible. For acute injuries, we also offer Orthopaedic Urgent Care at 3 locations for convenient walk-in services.
