Step 1
Submit the required information to create your online portal access. You will receive a confirmation email (usually in the same business day)
Step 2
After logging into your portal, navigate to “Appointments” in the left menu bar to complete your appointment request.
Important: This is for new and prospective patients to enroll in our practice. Please do not fill the pre-registration if you are already a patient in our practice. If you need access to the web-portal or if you are having trouble logging in, please contact the practice at 419-963-4870
Login to your existing patient portal and navigate to “Appointments” in the left menu bar to complete your appointment request.
