Medical Records Release

Need access to your medical record (labs, visit notes, etc.), all of this and more can be found by logging into your Patient Portal. Our office first has to have your email address so that we can web enable you. There is no charge and you can access it 24/7.

If you need us to send your medical record to someone representing you or to another physician office then please complete this Medical Record Release Form and send to us by selecting the send button below. Please note that there will be a charge from ScanStat/Datafile for preparing and sending medical records.
