Referring Providers

Thank you for your referral to the Orthopaedic Institute of Ohio! We accept patient referrals through several options:

1. Electronic referrals through your EMR/EHR system (preferred)
2. Download our referral pdf form here and fax it to 419-224-0015. We will then call the patient to schedule within 2 business days.
3. Call us: Our main scheduling line: 419-222-6622, Option 1

We take great care in scheduling referrals as soon as possible. For acute injuries, we also offer Ortho Urgent Care at 3 locations for convenient walk-in services.
