In light of the recent changes to COVID-19 policy announced by Governor Mike DeWine, we have revised our office attendance policy. Patients may no longer bring visitors along to appointments at our offices. Exceptions will only be made for patients who are minors or who require assistance due to age or disability. Patients who require assistance may be accompanied by at most one guardian or caregiver.
We continue to require all patients, visitors, and staff to wear face coverings as ordered by the State of Ohio. Exceptions will only be made for those who are unable to wear a face covering due to a medical condition and can provide supporting medical documentation.
Thank you for your understanding and cooperation as we work to keep our patients, staff, and community safe and our offices open so that we may continue to provide essential care to those who need it.

All patients may now be accompanied by up to one visitor at their appointment. To avoid crowding in waiting rooms, the allowed visitor may be asked to wait outside or in their car until the patient is called back to the exam room. The patient will then be responsible to notify the visitor when they are taken back. All visitors accompanying a patient must have their temperature taken and answer the COVID-19 screening questions as well.

In order to protect our patients, staff, and the communities that we serve, OIO requires that all patients, vendors, and other visitors to our facilities wear masks effective immediately. This is in addition to our requirements that our employees wear masks and monitor temperatures in order to prevent the spread of COVID-19. Exceptions will only be made for children under 2 years old and those who are medically unable to wear a mask.
As before, we ask that if you are exhibiting symptoms of or have been exposed to COVID-19 please stay home. If you are unable to attend an appointment for any reason, please contact us at (419) 222-6622 to cancel or reschedule.
We thank you for your help in working to make our offices a safe place for every patient so that we may continue to serve our communities with compassion and excellence.
